Try going to Admin > Content Management > Content and using the filters for Book Pages to locate them.
You can then Right click and Copy Link Address to get the address of the pages now.

The path to one of the the pages that is missing is http:/chordsmen.groupanizer.com/contract and that path was never copied over from the "old" server. The other paths are:
http:/chordsmen.groupanizer.com/meeting-minutes
http:/chordsmen.groupanizer.com/organizational
http:/chordsmen.groupanizer.com/singing

So, if you go to the option that Ian mentioned above, you should be able to find the page, and then change the parent ot an existing one to get them visible again. Once there, you might need to re-create the top level stuff as the UI has changed and I wonder if there is extra info that is needed now to show them to the users.
