Saved changes to images not appearing

I've noticed lately that some of the changes I've been making to areas involving images tell me that "the changes have been saved".   But when I view the changed page, the changes have not been made, or a different image appears.
Here's a specific example:  I recently tried to change the picture on one of the accounts of my quartet - from an image of our business card, to a 65x65 image of the quartet.  I deleted the business card image...successful.  I then uploaded the new image and saved the change...successful (or so i was told).  Now when I look at the profile of that member, the picture is of the business card.
I have tried flushing all the caches in between the delete and upload...same results. 
Has anyone else experienced this?  Is it a bug, or am I overlooking something very obvious?
Any help would be appreciated, this is becoming frustrating.
